The Good Shepherd Health Center therapy team is made up of highly qualified physical therapists, occupational therapists, and speech-language pathologists.  This team provides therapy services to individuals at Good Shepherd and even makes “house calls” to those in the continuum including, Cornerstone assisted living facility, Kentucky Ridge assisted living, Shalom Towers, and the Manor.  Each patient’s treatment plan is customized to meet their individual needs.

Long-Term Care Resident Rehabilitation Services

Good Shepherd’s therapy team designs programs to address the specific needs of each individual patient.  Our long-term care residents appreciate the advantage of convenient, on-site physical, occupation, and speech therapy services.  At Good Shepherd we strive to maintain each patient’s highest level of function so that they may have as much independence and dignity as possible.

Homeward Bound  (Short – Term Rehab)

Often times following a hospitalization, patients require further rehabilitation before transitioning home.  Good Shepherd’s Homeward Bound short-term rehabilitation program is the solution.  Not long ago, Medicare allowed patients to spend more time in the hospital to recover from an illness, surgery, or injury.  Now patients are discharged from the hospital quicker and often times sent to a long-term care facility for therapy and recovery.  To quality for skilled nursing care, you must be medicare eligible, you must have a qualifying hospital stay of 3 nights, and your physician must feel that you need daily skilled care given by, or under the direct supervision of skilled nursing or rehabilitation staff.  At Good Shepherd Health Care Center we provide in-house therapy to meet your individual needs.  We focus on returning individuals to their highest functional level with the ultimate goal being discharge to home when medically indicated.  Our therapists routinely complete home safety evaluations prior to discharge home in order to personalize recommendations as they relate to your home environment.
